pain mgt with the nurse = just a big pharmacy bill, a facility fee from the hospital that owns your practice, and your office visit because your out of network with everyone
no doubt you would shake me down for a copay for both the office visit and use of your waiting room if you werent retired and i was unfortunate to need some pain meds
the shake dowen would take longer than your assesment,

"i need both copays now"
"all my other drs just charge me one copay they accept my insurance and they dont bill me at home"
"do you want the drugs?"
"ya"
"give me both copays now and pay your bill when i send it or i will put you into collections again"
"here" hands the nurse a stack of sweaty ones for the two copays
"ok, see you in two weeks for your followup"
"whats the followup for?"
"do you want more drugs?"
"ya"
"then see you in two weeks for the followup and dont forget your two cash copays and pay the bill that i am sending out today. next!"
